Heat blueberries, water, sugar, and 2 sprigs thyme in a saucepan to a boil over high. Reduce heat to medium-low, simmer until sugar dissolves and berries burst, crushing any remaining whole berries with a potato masher, 10 minutes. Let blueberry-thyme syrup cool to room temperature; discard sprigs, transfer to a pitcher, and chill until ready to serve.
Stir tequila and lemon juice into blueberry-thyme syrup; divide among four glasses filled with crushed ice and garnish with thyme sprigs, reserved blueberries, and lemon slices.
